- 30 minutes: $43
- 45 minutes: $60
- 60 minutes: $75
- 90 minutes: $93
- Special Hot Stone Massage: $85
The above prices include applicable taxes.
Steps to Make a Claim
- Do you need a doctor’s written referral for insurance coverage? Contact your human resource department or your health care insurance company to find out.
- Explain your symptoms to your doctor. Say that you would like to treat them with massage and ask for the referral.
- After your massage I will give you a receipt containing my registration number, the date, the cost and the type of treatment I provided.
- Obtain and fill out a claims form from your place of employment.
- Submit the form with the doctor’s note and my receipt enclosed. Your insurance company will forward you a refund and statement.
