Privacy
All health information that you share is kept confidential. I am required to get your permission to share any of your information and to let you know exactly who I will be sharing it with. This would only occur if discussing your case with another health care provider, insurance company representative, or your family member. There are some extenuating circumstances that I am required to report information without your consent to the appropriate authorities, such as:
- When certain types of abuse are suspected
- To reduce a significant risk of serious bodily harm to a person or the public
- To assist health researchers for research, as long as strict privacy requirements are met
- For risk management and legal purposes
- For administration or enforcement of laws about the practices of health care providers, such as to all the College of Massage Therapists of Ontario and other legal bodies to regulate the practices of health care professionals
- For the purpose of a legal proceeding of complying with a court order, or other legal requirement
You are able to see all of your health records and make any changes you would like to your case history form.
